Supporting a Family Through End-of-Life Care

A real-life example of how personalized support can help an individual and their family navigate an end-of-life transition with dignity, compassion, and greater peace of mind.




THE CHALLENGE


When caring for someone becomes too much to navigate alone

A woman reached out to me for support as her 53-year-old brother was nearing the end of his life following an unsuccessful treatment for lung cancer. He was receiving palliative care and could no longer safely be left alone. He needed help with many everyday tasks, including walking and transferring, while his wish was to remain at home for as long as possible. His sister wanted to honour his wishes while also managing the growing responsibilities of caregiving.


She needed support — not only with caring for her brother, but with navigating everything that came with this transition.



THE SUPPORT


A plan built around what mattered most

After our initial consultation, I took the time to understand their situation, needs, values, and wishes. Together, we created a flexible support plan that could adapt as their needs changed.


The plan included:


  • Respite & Family Support

Providing approximately three hours of support each week so his sister could shop, run errands, attend to personal needs, or simply take a break.


  • Practical Daily Support

Assistance with transfers, toileting, preparing food and drinks, and other everyday needs.


  • End-of-Life Planning

Helping document final wishes, explore funeral planning, and prepare for the final days and hours.


  • Legacy Planning

Supporting him in creating personal video messages for people he wanted to leave something meaningful behind for.


  • Care Coordination

Helping his sister coordinate medical support in the home, prepare for hospice care, and connect with appropriate services.


  • Information & Navigation

Providing guidance around paperwork, documentation, referrals, and available resources as requested.



THE TRANSITION


When the plan needed to change

As his condition progressed, remaining at home was no longer possible and hospice care became necessary. He moved to hospice, where he spent his final three weeks. I continued to visit daily, helping coordinate his care and advocating for support that honoured his wishes, dignity, and values. When his final days arrived, his family chose to remain by his side around the clock. I continued to be available from a distance whenever support was needed.


The goal was never to dictate the journey — it was to help the family navigate it in a way that honoured who he was and what mattered to him.



THE OUTCOME


Support that continued beyond his passing

After he passed, I continued to support the family through the practical aspects that followed. I helped coordinate services, navigate required paperwork and documentation, and provided informational support whenever the family needed it. Most importantly, the family had support throughout a deeply difficult transition — from caregiving at home, to hospice, to the final days, and beyond.



WHAT THIS SHOWS


End-of-life support isn't only about the end.

It can be about how we live, how we care for one another, and how we honour what matters most.


My approach is rooted in essence-based planning — taking the time to understand a person at their core: their values, preferences, relationships, spirit, and what gives their life meaning.


From there, support is guided by who the person is, rather than simply by the circumstances they are facing.


An Unconditional Presence

Throughout this journey, I offered an unconditional presence — a steady source of compassion, information, practical support, and guidance without judgment. The family was able to choose the level of support that worked for them, with an affordable package based on the hours and services they requested.


Every journey is different.

Every person is different.

Support should be, too.
